IMO dragons (and gods) are the size they imagine themselves to be. The size that fits the plot. They don't need consistency in size, any more than we need consistency in the angle between hand and arm - it's variable and totally under our control.
Giants I'm less sure about.
And on the Hero Plane, I'd expect size to be dependent on magical power. Of course, the idea of a dragon (or god) who's totally in the mundane world, not in the hero plane even a little bit, is unlikely, so they're heroic size even when we're seeing them on the mundane.
