Specific heroquests. The most famous is the Hill of Gold, for Yelmalions.

My guess would be they are summoned by later worshippers, either bodily or via incarnation ("Heroforming").

To be a Hero you need to succeed an Ageless quest, find a "new" magic (i.e. previously unknown to your people), and be worshipped. The exact order is unimportant. Arcane Lore suggests that these all need to be completed prior to Death, but I suspect that some variation is possible, most likely in gaining worshippers after passing.
