Of course, this is a modern westerm idea. The concept of doctoring in the
'east' is and was wildly different. There (AFAIK) you paid the doctor while
you were well, and it was their job to keep you well. When you got sick, you
didn't pay them as they hadn't done their job. The Dr made many more housecalls
under this regime that they do now :-) Once/yr = no food for Dr!
