I always assumed that detect Enemy was a telepathic spell that picked up
someone's awareness of the caster. So random guards do not register, but
guards who have an immediate reaction to a broken twig would, even if they
don't know who they are searching for.
Similarly, a random ambush would have to be in line of sight for the
ambushers to register on the spell.
